Step 1: Complete the Form

Fill out the required enrollment form using the link below. (This pdf is fillable on a computer, but only printable on a mobile device.)

Step 2: Upload Documents:

Upload the completed enrollment form, along with your student's immunization records or a valid exemption form (upload pdf or jpg of documents using form below).

Step 3: Pay Fees

After your documents are successfully submitted, you will receive a confirmation email with clear instructions on how to pay the required registration fees.

Once you have received your confirmation e-mail, you are ready to finalize your enrollment. Enter the seating fee amount in the field below and click "Submit Seating Fee." A secure payment window will appear to walk you through the transaction.
